The Emperor must be the ML. The girl looks like she is wearing the wedding clothes for the royal family in the cover.
She was meant to be Sinwoo's from the start, and with plots like that the FL tends to end up with her fated one. Plus from the raws it does not look like she deliberately chose to reject him but she could not get married because of the villains.
As for the royal clothes on the cover.. I have an hypothesis, that our FL is in fact someone with real powers that connects her with the deceased queen. She seems to speak things when panicked.. things that somehow seem to be what the queen would have replied to the king's questionings.
But from the raws I have seen. It looks like the female lead actually has feelings for the emperor.
She seems excited to be with him. Spending time with him. Being occasionally intimate with him.
