You need to create a custom page, look under specialty pages here:
themesbyeris.tumblr.com/links (link 1)

Then you need to insert a code. This is the tricky part. You need to find a pre-made code for a follow forever page. In order to do that, you go here:
theme-hunter.tumblr.com/tagged/blogroll (link 2)

The page I linked to provides several posts. Each post contains a preview and a code for an all-in-one page. You click the preview in each post until you find a preview you want to replicate, you click on code, copy the code and paste it in the box like they've shown you in (link 1)

Instructions:
copy the whole code from the paste bin then insert it or paste it in the box of the tumblr page you want to create EXACTLY LIKE THEY SHOW YOU IN LINK 1
