I don't get why everyone is so confused about this. Everything makes sense here. Cha Jinwoo is called 'immortal' because he is human who has an ability to reincarnate. But destiny always leads him to tragic love where one of the lovers has to sacrifice himself. Jia was his younger sister but died at age of 5. God made him forget this event and pretended to be her in order to watch him closely. In all Cha Jinwoo's previous lives either he or his lover dies and that's why he wants to save Oh Yeonsoo so desperately. He wants them to have a love without sacrifices at least in one of his multiple lives. The ending is 'open' but it's clear that Cha Jinwoo overcame fate and lives with his beloved one happily ever after.
